RELICENSURE

This page is designed to assist our EMS providers and their Agency Coordinators with provider relicensure. If you are not in the Will/Grundy System then use this link

There are a some key factors to remember when it comes to relicensure:

1. Make sure the Will/Grundy EMS System office has your current address at all times.

2. Always keep copies for your own records. (As it is ultimately your responsibility.)

3. Never wait till the last minute. Check your CE every 3 months and track your hours.

4. Always obtain details of your classes, such as topics covered, site codes and signatures.

5. Use the FORMS below to help guide you through the process.
