RELICENSURE

This page is designed to assist our EMS providers and their Agency Coordinators with provider relicensure. If you are not in the Silver Cross System then use this link

There are a some key factors to remember when it comes to relicensure:

1. Make sure the Silver Cross EMS EMS System office has your current address at all times.

2. Always keep copies for your own records. (As it is ultimately your responsibility.)

3. Never wait till the last minute. Check your CE every 3 months and track your hours.

4. Always obtain details of your classes, such as topics covered, site codes and signatures.

5. Use the FORMS below to help guide you through the process.

The State of Illinois will institute licensing and relicensing fees effective April 1, 2012,

as well as a fee to request a duplicate copy of your license.

The process for submitting payments will be listed here as soon as its published.

All relicensure policies will be revised once we receive the renewal fee instructions from IDPH.
